It has been brought to our attention that there is some confusion about the use of the term "electronic health record" in the "Point."  The accepted terminology is still electronic medical record (EMR) for an electronic record for just one agency (including all entities such as other hospitals, VNAs etc.) and electronic health record (EHR) for electronic records that are shared between agencies.  Other terminology, such as electronic patient record (EPR) is not accepted by the NAHIT (http://www.nahit.org/pandc/press/pr5_20_2008_1_33_49.asp). Using EPR, or confusing the EMR with the EHR, can only provide points of confusion for learners and others. The ultimate goal of the US government is that EMRs will morph to EHRs.
